Introduction

These Web Form Terms of Use (“Terms”) govern your access to and use of the Family DOORS Everyday web form (“FDE”). By accessing or using the FDE, you agree to be bound by these Terms. If you do not agree to these Terms, please do not use the FDE.

Purpose and Scope

The Family DOORS Everyday is a free, online tool designed to assist practitioners in providing screening and support services for families. The FDE is intended for use by registered practitioners only.

As a community services organisation, we strive to provide free access to Family DOORS Everyday to as many practitioners as possible. However, due to system limitations and resource constraints, we have implemented certain usage limits to ensure the tool remains accessible and sustainable for all users.

Limitations:

  • Daily Screening Limit: There is a daily limit of 10 unique client links that can be sent from your account. This limit is in place to prevent overuse and ensure the tool remains accessible for all practitioners.
  • Excessive Usage: While there is no strict limit on the number of screenings you can perform, excessive usage may impact the tool’s performance and accessibility for other users.

By using Family DOORS Everyday, you agree to:

  • Use the tool responsibly and within the established limits.
  • Avoid excessive usage that may negatively impact other users.

Access and Use

Registration: To access the FDE, you must register as a practitioner user. You are responsible for maintaining the confidentiality of your login credentials.

Permitted Use: You may use the FDE only for the purposes of screening individuals for potential risk factors related to family violence. You may not use the FDE for any other purpose, including, but not limited to, commercial purposes or unauthorised access to data.

 

Prohibited Activities: You may not:

  • Access or attempt to access any part of the FDE that is not intended for you;
  • Modify, copy, distribute, transmit, display, perform, reproduce, publish, license, create derivative works from, or sell any information or content obtained from the FDE;
  • Reverse engineer, decompile, or disassemble the FDE or any part thereof;
  • Attempt to circumvent any security measures in place on the FDE.

Data Privacy and Security

  • Data Collection: We collect the following information:
  • Practitioner registration details (user profile)
  • Client information entered into the Form
  • Data Retention: We retain practitioner registration details for administrative purposes. Client data is processed and deleted immediately after screening.
  • Data Security: We have implemented reasonable security measures to protect your data from unauthorized access, disclosure, alteration, or destruction. However, no security system is impenetrable, and we cannot guarantee the absolute security of your data.  

Data Sharing

We may share your information with:

  • Other RASA entities and the authors of DOORS for research and quality improvement purposes.
  • Government agencies or law enforcement authorities as required by law.

Your Rights

You have the right to:

  • Access and correct your personal information.
  • Request the deletion of your personal information.
  • Object to the processing of your personal information.
  • Withdraw your consent to the processing of your personal information.

Data Retention

  • We retain practitioner registration details (user profile) for the purpose of providing access to the FDE and for administrative purposes.
  • We do not retain any client data or their responses submitted through the FDE. All client data is processed and deleted immediately after the screening is completed.

Disclaimer

  • No Warranties: The FDE is provided “as is” without any warranties, express or implied, including, but not limited to, warranties of merchantability, fitness for a particular purpose, or non-infringement.
  • Limitation of Liability: In no event shall we be liable for any indirect, incidental, special, consequential, or exemplary damages, including, but not limited to, damages for loss of profits, goodwill, use, data, or other intangible losses, arising out of or in connection with your use of or inability to use the Form.

Governing Law and Jurisdiction

These Terms shall be governed by and construed in accordance with the laws of the State of South Australia, Australia. Any dispute arising out of or in connection with these Terms shall be submitted to the exclusive jurisdiction of the courts of South Australia.

Changes to These Terms

We may update these Terms from time to time. Any changes will be effective upon posting on the FDE. Your continued use of the FDE after such changes have been posted constitutes your acceptance of the modified Terms.

Additional Terms

  • The Family DOORS Everyday Form is a free service that can be accessed through a web browser.
  • Family DOORS Everyday is a free tool designed for small-scale usage by practitioners. Excessive usage will impact the tool’s performance and use by others.
  • Organisations seeking a dedicated, customized screening solution are encouraged to explore developing their own digital version of the DOORS.
  • We do not provide technical support for the FDE. If you encounter any issues, please consult our online resources or contact your IT support.

Effective Date: These Terms of Use are effective as of 1 October 2024.

By using the Family DOORS Everyday Form, you agree to be bound by these Terms.
